Position Summary: Case Managers play a crucial role in engaging patients and their families to create effective discharge plans. They ensure that the discharge process aligns with the patient's clinical progress.

Key Responsibilities:

1. Oversee clinical care, including discharge planning, medical necessity assessments, and length of stay management.

2. Collaborate with healthcare providers, nursing staff, ancillary personnel, patients, and their families to achieve optimal clinical outcomes.

3. Maintain clear, comprehensive, and precise documentation of all interventions and assessments.

4. Adhere to regulatory standards, prioritize patient safety, and engage in performance improvement initiatives.
