Position Summary:

The Program Manager for Case Management Services plays a vital role in the leadership team at Great Bay Services, overseeing the case management operations in Sanford, Maine. This position entails managing and supervising the case management team, supported by a contracted clinical supervisor.

The Case Management program is designed to assist individuals with disabilities in connecting with their communities, promoting independent living. Clients aged 18 and older will engage in regular meetings with their case managers to collaboratively develop personalized annual plans. Our services encompass advocacy, planning, and coordination tailored to each client's needs.

Key Responsibilities:

  • Supervise the Case Management team in the Sanford office
  • Evaluate and monitor the performance of case management activities
  • Conduct regular supervision sessions with individual case managers and group meetings
  • Recruit and train new case managers as necessary
  • Manage and respond promptly to referrals for case management services
  • Participate in supervisory meetings and training sessions as required
  • Ensure case managers are informed of current guidelines and resources
  • Collaborate with case managers to enhance service delivery quality
  • Advocate for and coordinate access to essential services such as medical care, housing, employment, and community involvement
  • Maintain comprehensive case records, including assessments and service plans
  • Act as Representative Payee for clients when needed
  • Develop expertise in relevant software and programming

Qualifications:

  • Bachelor's Degree or higher in social services, public health, or a related field
  • Preferred qualifications include Registered Nurse or Licensed Independent Clinical Social Worker
  • A minimum of four years of experience working with individuals with intellectual disabilities or autism
  • Demonstrated experience in team leadership
  • Familiarity with the public education system in Maine, including flexible funding options
  • First Aid and CPR certification (training available for those without certification)
  • Valid Driver's License and mandatory auto insurance coverage
  • Must exhibit high ethical standards, motivation, reliability, and organizational skills
