Job Responsibilities:
- Conduct Initial and continuing assessment of each participant and development of a written plan or care.
- Provide restorative and supportive nursing as indicated by the participants’ treatment plan.
- Monitor, administer, and record prescribed medications.
- Monitor and record vital signs.
- Supervise personal care given by Program aide.
- Maintain participant’s health record which includes: quarterly progress notes, records of medications and treatments administered, progress notes, flow sheets.
- Liaison with PCP, caregivers and family members.
ADHC centers provide a range of services, including:
Nursing services, such as wound care and medication management
Rehabilitation therapies, such as occupational, physical, and speech therapy
Psycho-social services, such as counseling and social work
Health monitoring and education
Nutritious meals and snacks
Structured activities, such as exercise classes, arts and crafts, and games
Socialization and companionship
