Cayuga Counseling Services, Inc. partners with the Onondaga County Probation Department to offer an important service in Onondaga County by operating an enhanced pre-trial release program. This program seeks to address the needs of defendants housed in the Onondaga County Justice Center who suffer with a serious mental health illness and possibly a co-occurring substance abuse disorder with the goal to have the defendant released and remain in the community as their case proceeds. Cayuga Counseling is seeking a Full-time Care Coordinator to facilitate this process. The Care Coordinator will perform the following duties:

  • Receive referrals from the Onondaga County Probation Department
  • Interview defendants housed at the Onondaga County Justice Center and complete appropriate assessments.
  • Upon release of defendant, identify and assist with establishing additional community related linkages to services.
  • Communicate with service providers and the assigned Probation Officer to ensure client participation and attendance
  • Document in a timely manner all contact with service providers and clients in the Onondaga County computer database
  • Track and compile program data
  • Complete client aftercare contact/referrals for three months following release from the program
  • Assist with Pre-Trial Release case manager duties as assigned.
  • Preform other duties as needed.

Qualified candidates will have: Bachelor's degree in social work or a related human services field; 3+ years experience with intensive case management/care coordination; the ability to develop and foster relationships; excellent verbal and written communication skills; prior experience with the criminal justice population is a plus.

The Agency offers:

  • 3 Weeks PTO in your first year, 4 weeks in your second year.
  • 48 Hours of Wellness time annually (4 hours accrue per month) that can be used any time you feel you need some time for yourself.
  • 10 Paid Holidays per year including 1 Floating Holiday.
  • 96 Hours of sick time annually (accruing 8 hours per month).
