Responsibilities:
  • Develop and implement personalized care plans to meet the unique needs of our residents.
  • Coordinate with our healthcare team to ensure seamless care delivery and effective communication.
  • Provide direct care to residents, including administering medications, performing procedures, and providing emotional support.
  • Document all information related to client care, including medical history, treatment plans, and progress notes.
  • Recognize changes in resident status and report to appropriate personnel.
  • Act as a liaison between residents, families, and physicians to ensure effective communication and care coordination.
  • Maintain timely physician orders and communicate with physicians regarding resident status.
  • Supervise paraprofessional personnel to ensure efficient and effective care delivery.
  • Participate in activities beneficial to the growth and development of Menorah Park.
Requirements:
  • Active LPN license in the state of New York.
  • Minimum of 1 year of experience in a long-term care or healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and efficiently in a fast-paced environment.
  • Proficient in electronic medical records systems.
  • Excellent organizational and time-management skills.
  • Ability to prioritize and multitask effectively.
  • Demonstrated empathy, compassion, and patience towards residents and their families.
