Responsibilities:- Develop and implement personalized care plans to meet the unique needs of our residents.
- Coordinate with our healthcare team to ensure seamless care delivery and effective communication.
- Provide direct care to residents, including administering medications, performing procedures, and providing emotional support.
- Document all information related to client care, including medical history, treatment plans, and progress notes.
- Recognize changes in resident status and report to appropriate personnel.
- Act as a liaison between residents, families, and physicians to ensure effective communication and care coordination.
- Maintain timely physician orders and communicate with physicians regarding resident status.
- Supervise paraprofessional personnel to ensure efficient and effective care delivery.
- Participate in activities beneficial to the growth and development of Menorah Park.
- Active LPN license in the state of New York.
- Minimum of 1 year of experience in a long-term care or healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work independently and efficiently in a fast-paced environment.
- Proficient in electronic medical records systems.
- Excellent organizational and time-management skills.
- Ability to prioritize and multitask effectively.
- Demonstrated empathy, compassion, and patience towards residents and their families.
