Job Summary:

Key Responsibilities:

  • Develop and Implement Care Plans: Work closely with our healthcare team to develop personalized care plans that meet the unique needs of our residents.
  • Coordinate Care Services: Coordinate and manage the delivery of care services, including medication administration, wound care, and other medical needs.
  • Communicate with Residents and Families: Provide excellent communication and interpersonal skills to ensure that residents and their families are informed and engaged in their care.
  • Collaborate with Healthcare Team: Work collaboratively with our healthcare team, including physicians, nurses, and other healthcare professionals, to ensure that residents receive comprehensive and coordinated care.
  • Document Resident Care: Accurately and thoroughly document resident care, including medical history, treatment plans, and progress notes.
  • Recognize and Report Changes in Resident Status: Recognize and report any changes in resident status, including changes in medical condition, behavior, or other factors that may impact care.
  • Act as Liaison: Act as a liaison between residents, families, and healthcare providers to ensure that care is coordinated and effective.
  • Maintain Accurate Records: Maintain accurate and up-to-date records of resident care, including medication administration, treatment plans, and progress notes.
  • Participate in Quality Improvement Initiatives: Participate in quality improvement initiatives to ensure that care is delivered in a safe, efficient, and effective manner.

Requirements:

  • Active LPN License: Must have an active LPN license in the state of New York.
  • Experience in Long-Term Care: Must have a minimum of 1 year of experience in a long-term care or healthcare setting.
  • Strong Communication Skills: Must have excellent communication and interpersonal skills.
  • Ability to Work Independently: Must be able to work independently and efficiently in a fast-paced environment.
  • Proficient in Electronic Medical Records: Must be proficient in electronic medical records systems.
  • Excellent Organizational Skills: Must have excellent organizational and time-management skills.
  • Ability to Prioritize and Multitask: Must be able to prioritize and multitask effectively.
  • Demonstrated Empathy and Compassion: Must demonstrate empathy, compassion, and patience towards residents and their families.
