Overview: The Client Care Coordinator plays a vital role in collaborating with individuals with intellectual and developmental disabilities (I/DD) to facilitate the necessary care and services that enable them to achieve their health, wellness, and personal aspirations.

Key Responsibilities:

  • Conduct thorough evaluations to determine the clinical and psychosocial requirements, preferences, and choices of individuals.
  • Identify and address health and safety concerns, as well as obstacles to treatment, including social determinants affecting health.
  • Work alongside an interdisciplinary team to integrate feedback into a comprehensive assessment and individualized Life Plan.
  • Create and uphold a personalized Life Plan that synchronizes clinical and non-clinical healthcare needs and services.
  • Oversee the execution of the Life Plan and provide necessary updates.
  • Incorporate health promotion strategies to support individuals in achieving and sustaining optimal health.
  • Facilitate access to chronic disease management resources.
  • Coordinate referrals to clinical and community services, ensuring effective planning and follow-up for transitional care.
  • Ensure access to long-term care supports and services.
  • Engage families and natural support systems in the care coordination process.
  • Guarantee that all services provided are culturally and linguistically appropriate.
  • Advocate for the individual’s needs and promote self-advocacy skills.
  • Utilize Health Information Technology for documentation and to enhance communication among the care coordination team.
  • Safeguard all health records and sensitive information in compliance with confidentiality standards and regulations.
  • Maintain adherence to all applicable state and federal laws, Medicaid regulations, and organizational policies.
  • Document all services rendered and maintain accurate records following established procedures.
  • Represent the organization in various committees and workgroups as needed.
  • Engage in ongoing training and professional development opportunities.
  • Perform additional duties as assigned.

Qualifications:

  • Ability to respond swiftly and effectively in crisis situations.
  • Basic proficiency in technology and understanding of health records.
  • Knowledge of ethical standards and professional boundaries.
  • Exhibit professional work habits, including reliability, time management, and accountability.
  • Some positions may require bilingual capabilities.

Education and Experience:

  • Bachelor’s degree with two years of relevant experience, or
  • A Registered Nurse license with two years of relevant experience, or
  • A Master’s degree with one year of relevant experience.

Working Conditions:

  • Ability to sit or stand throughout the workday.
  • Proficiency in entering data and documentation into a computer.
  • Willingness to travel as required.
