Job Title: Direct Support Leader (DSL)
Summary: Works productively as a team member and provides support to DSS to ensure quality services for the individuals we serve. Carry Out the mission to provide quality care, life enrichment, indefinite possibilities and a family atmosphere to those who have been diagnosed with disabilities.
Must plan and carry out services and support in relation to all phases of the daily living/services of the group you are supervising. Must also be able to be responsible for the supervision, training and documentation of the pod you are assigned to.
Below are the components you as leaders will be responsible for:
Training of Company
- Job description
- Company Policy
- Code of Conduct and Rules and Regulation
- Policy and Procedure
Paperwork
- Make sure all paperwork is retrieved and turned in prior to working in the home
- Make sure all signature for training has been received
- All Reference sheets are completed and entered into the system for expiration dates
Scheduling
- Schedules are consistent with client needs
- Availability
- Training Schedules
Clients ISP’s
- Training on all aspects of ISP
- Behavior support if needed
- Documentation
- Outcomes and Activity logs
Communication
- Daily log books in every home for clients and staff
- Leaders at least Weekly checks/also whenever working
- Requirements for Emergency communication calls
- Communicating Appointments, maintenance issues, client request
- UI processing
- Med Errors/ refills, house supplies, petty cash
Meeting
- Monthly house/building meeting organized
- Address any issues
Budgeting
- House bills, personal needs, sent to payee
- Budget and monitor petty cash
- Monitor spending with petty cash logs
- Collect receipts and turn in weekly
Medical Coordination
Schedule monthly appointments
- Call in medication
- Count controlled substances
- Make staff accountable for signatures
- Med error reporting
- Nurse interaction
Activity Involvement
- Scheduling group activities
- Arranging rides
- Budgeting for events
Paperwork
- Collect the paperwork from your homes weekly
- Check to make sure they are correctly filled out
- Call the staff in to make corrections
Routine
- Create a routine at each house based upon clients likes as grid line to go by
- Have staff who work help with ideas
- Learn what works best to support the client
- Alternate days so no one is stuck with all the work
Crisis Intervention
- Training
- Identify issues

Assignment Log
- Lets parents, clients, sass, and staff know who they should contact with question

Actively participating and supervising the essential duties and responsibilities include the following:
Direct Service- A family oriented team of trained professionals committed to making sure that the individuals we serve have the tools they need to reach their full potential while maintaining their independence. Our loving and compassionate team will strive to provide excellent support and care to enhance lives and achieve new goals.
When working at a vocational site, staff are required to provide support to prepare individual’s development of vocational, recreational, and social skills, to apply them to various situations according to their Individual Service Plan/ISP.
Ensures that each individual’s needs are met, including personal care, mealtime, medication administration/assistance, equipment adaptations, daily household chores and errands, vocational opportunities, and transportation.
Assist in developing a positive living and work environment by being proactive and preventing behaviors.
Supports each individual’s physical, social, emotional, and intellectual growth.
Communication: Encourages and maintains good communication with co-workers, families, school, and work settings, neighbors, and community contacts.
Represents C&J Tender Heart R.S. in a positive and professional manner.
Recordkeeping: Maintains complete and accurate records regarding individual’s progress, health problems, services, program implementation, incident reports, financial transactions, and household operations. Comply and submit information and reports specific to the employee's department.
Must be able to handle money and financial transactions, such as balance a check, a checkbook, fill out expense forms and maintain receipts and records.
Housekeeping: Perform housekeeping and maintenance tasks as required
Ensures sufficient food and household supplies are kept on hand, as appropriate to setting and reports other needs as necessary. Effectively and respectfully communicates with individuals within the agency as well as those in the community.
Staff Development and Training: Conduct staff meetings and in-service training to continue to develop and share job-related skills. May assist in the training of other staff.
Scheduling: Direct Support Specialist DSS will be scheduled with the flexibility between work sites to best meet the needs of the individuals receiving services. Staff will be trained and required to work in any area they are assigned to.
Medical Appointment DSS Additional Responsibilities: Schedules, transports, and attends individual’s medical appointments, tests, and/or procedures. Schedules follow-ups as needed.
Identifies and organizes items needed for approaching appointments, including documentation, premedication, known consents, and ensures availability and completion prior to appointments.
Follows all C&J Tender Heart R.S. transportation policies and procedures whether transporting in a C&J Tender Heart vehicle or in the employee’s personal vehicle.
Reports any maintenance issues or concerns.
Report all accidents and traffic violations.
Complete required daily paperwork.
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The employee must be at least 18 years of age.
Education and/or Experience: High school diploma/ged required; previous experience in a care giving.
Language Skills: Ability to read and interpret documents such as safety rules, programs, medication charts, and procedure manuals. Ability to write routine reports, individual’s programs, and correspondence. Ability to speak one on one, with families/others in the community and before co-workers in a small group setting.
Mathematical Skills: Money management, ability to add, subtract, multiply, and divide.
Reasoning Ability: Ability to solve practical problems in a variety of situations using a common approach. Ability to make good judgments.
Other Requirements and Skills: The employee is responsible for maintaining a valid driver’s license and a safe driving record to maintain insurability. The employee is responsible for successfully completing all training required to maintain compliance with regulations. The employee must maintain First Aid/CPR certification and successfully complete and maintain the medication administration training required for the area in which they work.
