Job Title: Emergency Community Support Provider

Supervised by:  Outpatient Director

Qualifications:

  1. Be 21 years of age.
  2. Have a valid Nebraska Driver’s License and be insurable, have a good driving record and be a confident driver.
  3. If diagnosed with a substance use disorder, individual must have a minimum of 18 months’ recovery/abstinence.  Must have the ability to pass an initial and ongoing random drug screenings.
  4. Have a Bachelor’s degree or higher in psychology, sociology, or a related human service field is preferred.  Two years of coursework in a human service field and/or two years of experience/training or two years of lived recovery experience with demonstrated skills and competencies in treatment with individuals with a behavioral health diagnosis is acceptable.
  5. Have a sophisticated set of interpersonal skills, including verbal fluency, interpersonal perception, affective modulation and expressiveness, warmth and acceptance, empathy and focus on others.
  6. Have the ability to display cues of acceptance, understanding and expertise in the first moments of interactions with consumers through verbal and non-verbal behavior.
  7. Possess the ability to form a working alliance with a broad range of clients.
  8. Be willing to become knowledgeable of supportive services and other referral agencies in the community to benefit clients, and develop other community resources.
  9. Have the ability to work with other agency professionals to meet needs and best interest of clients, serving as a liaison between agencies.
  10. Exhibit excellent written and oral communication skills.
  11. Able and willing to practice strict confidentiality guidelines following 42-CFR and HIPAA regulations.
  12. Be resourceful, confident, assertive, have strong, appropriate boundaries, and be able to problem solve in a variety of situations.
  13. Must be able to work effectively as part of a team and independently, and able to discern which skill is needed in a given situation.
  14. Possess good stress management skills.

Duties and Responsibilities:

  2. Maintain a caseload of up to 15 emergency community support clients.
    1. Complete orientation and intake documentation with all clients within established timeframes.
    2. Complete authorizations for services through the Centralized Data System (CDS).
    3. Complete an initial needs assessment with each client within required timeframes.
    4. Complete an initial service plan with each client and update within established timeframes.
    5. Meet with each client as is necessary to meet their needs so that the emergency necessitating care has resolved.
    6. Complete contact notes within established timeframes.
    7. Maintain appropriate releases of information to effectively coordinate services for the client.
    8. Provide mental health board and probation/parole/drug court updates, as applicable.
    9. Monitor client progress and barriers.  Act as the central hub of the client’s treatment team by maintaining ongoing communication with all providers regarding the client’s progress and barriers in their treatment. 
    10. Complete an aftercare plan within two weeks prior to discharge.
    11. Upon the client’s discharge, complete a discharge summary established timeframes.
  3. Provide direct rehabilitation and support services to each client in the community with the intention of supporting the individual to maintain abstinence and prevent exacerbation of illness and admission to higher levels of care.  Areas of assistance may include;
    1. Providing referrals and coordinating services with other providers and agencies in the community.
    2. Assisting clients in accessing available resources.
    3. Helping clients obtain and maintain safe and affordable housing.
      1. Actively coordinate with Region 4 housing program regarding any clients receiving Region 4 housing assistance.
    4. Providing vocational and educational assistance.
    5. Providing transportation.
    6. Attending court appointments for support and encouragement.
    7. Attending psychiatry and medical appointments to assist the client in advocating, communicating, understanding, and following through with recommendations.
    8. Assisting the client in obtaining medication and maintaining medication compliance.
    9. Coordinating services and providing support for clients in crisis.
    10. Teaching skills including communication, budgeting, coping, relapse prevention, social, independent living, activities of daily living, leisure, decision making, and healthy meal planning.
    11. Wellness Recovery Action Planning.
    12. Providing motivation and support for healthy life changes including diet and exercise.
    13. Building a healthy social support network.
    14. Finding a spiritual community.
    15. Finding a recovery community and/or a sponsor.

                                                                                                  
Probationary Period:  6 months
