Bereavement Services Coordinator

St. Luke's Hospice is a highly rated program serving the Lehigh Valley for many years. This support to families at end of life is one where you can make a lasting impact and offer comfort and support. We are looking for a full time social worker interested in Hospice Grief and Bereavement services. We have a strong program and a great team. Experience in Hospice or Grief and Bereavement preferred.

  1. Develops and Coordinates the Bereavement Program, including:
  2. Attendance at Case review Team meetings for ongoing assessment and development of the Bereavement Plan of Care for individuals grieving loss of Hospice patient.
  3. Creation and distribution of monthly bereavement mailing.
  4. Workshops, Support groups and other grief-related support/outreach programs to the community.
  5. Trains and supports and supervises volunteers involved in bereavement services.
  6. Schedules, plans, evaluates and participates in support groups, workshops and grief
  7. related support/outreach programs in the community.

  1. Delivers Counseling services, including:
  2. Telephone calls to the bereaved.
  3. Individual and family short term grief counseling, in their home or at office.
  4. Referral to community professionals for long term counseling services and crisis management assistance.

  1. Plans and implements new programs for the hospice bereaved and the community.

  1. Educates medical and lay community on grief process and supportive measures.

  1. Assures that high standards of bereavement care are maintained through satisfaction surveys, audits and benchmarking.

  1. Supports hospice team in dealing with multiple losses.

  1. Participates in regional bereavement programs.

  1. Is available for emergency on-call service.

Medium Work Exertional activity

  1. Occasionally lift and/or carry 50 lbs

  1. Frequently lift and/or carry 25 lbs

  1. Being able to frequently lift or carry objects weighing up to 25 pounds is often more critical that being able to lift up to 50 lbs at a time

  1. Stand and/or walk at least 6 hours in an 8-hour work day

QUALIFICATIONS

(MINIMUM)

EDUCATION:

Masters level prepared in counseling psychology, divinity, education or social work with depth of knowledge and experience in bereavement or mental health related field. LCSW, LPC preferred.

TRAINING AND EXPERIENCE:

Background experience in Hospice and Bereavement care, counseling, group facilitation and psycho-education very useful.

Spanish Speaking Highly Desired.
