Role Description

This is a full-time on-site role for a Social Services Specialist at New Health Partners in Tomball, TX. The Social Services Specialist will be responsible for providing social work support in clinics, human services assistance, case management, and community outreach. They will work closely with individuals and families to assess their needs, develop care plans, and connect them with appropriate resources and services.


Qualifications

  • Social Work and Case Management skills
  • Experience in providing social services and community outreach
  • Strong communication and interpersonal skills
  • Empathy and compassion for individuals in need
  • Knowledge of local resources and support services
  • Ability to work collaboratively in a multidisciplinary team
  • Bachelor's degree in Social Work, Human Services, or related field a plus
  • Experience in healthcare or insurance industry is a plus
  • Must be bilingual in Spanish and English
