Role Description

This is a full-time on-site role for a Social Services Specialist at New Health Partners in Tomball, TX. The Social Services Specialist will be responsible for providing social work support in clinics, human services assistance, case management, and community outreach. They will work closely with individuals and families to assess their needs, develop care plans, and connect them with appropriate resources and services.


Qualifications

  • Social Work and Case Management skills

  • Strong communication and interpersonal skills
