Primary Duties:

·         Provides quality diabetes self-management education and medical nutrition therapy based on assessed needs in individual and group classes which are in-person or via telehealth.

·         Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient' assessed needs and goals and promotes successful self-management.

·         Conducts MedStar Diabetes Boot Camp visit one and visit two, follows the patient until s/he is transferred to the Virtual Clinic, uses MedStar Diabetes Medication Algorithm to perform shared decision making and to collaborate with Boot Camp NPs in the development of a diabetes medication plan that is patient centric.

·         Collaborates, advocates and confers with nurse practitioners, primary care providers, endocrinologists and other diabetes stakeholders in developing patient centered diabetes plan.

Qualifications:

·         Bachelor’s degree in Nutrition/Dietetics or higher required.

·         Registered Dietitian license - Registered Valid license based on location of practice as a Registered Dietitian required.

·         1-2 years related experience as a registered dietitian required.

·         Diabetes self-management education and support experience preferred.

·         Certified Diabetes Care and Education Specialist - CDCES Certified Diabetes Care and Education Specialist Upon Hire preferred.

·         CDE - Certified Diabetes Educator Certification If not already DCES or BC-ADM will sit for the certification examination within 24 months of hire date required.
