Wyndemere's 5-star skilled nursing facility is seeking a Director of Admissions and Social Services for a small, 56 bed life-plan senior community. This role coordinates and supports all social services related functions within the Social Services Department in accordance with current federal, state, and local standards. Assesses, plans, implements, and evaluates the social, emotional, and psychological needs of sub-acute patients. Coordinates all Health Center admissions and discharges through review with patients and families.


ESSENTIAL JOB DUTIES

SOCIAL SERVICES:

  • Has oversight and responsibility of the assessment, support, and discharge planning for the patients in the Health Center. Identifies and assists with mood, behavior, and psychosocial well-being issues for residents.
  • Maintains repeated contact with patients and their support systems to provide comprehensive updates on patient progress toward goal achievement. Provides ongoing communication with family/patient regarding discharge needs.
  • Utilizes problem-solving skills to assist patient and family in identifying potential barriers to discharge. Provides education, resources and training necessary prior to discharge from the facility.
  • Educates families and patients on the general criteria for Medicare, Medicaid, Hospice benefits, and insurance coverage. Identifies potential financial barriers and collaborates with family and members of the interdisciplinary team to make recommendations for treatment and discharge planning with consideration to financial status, diagnosis, and discharge needs.
  • Communicates daily with nursing and the team regarding admissions, discharges, and transfers in order to assist with Residents' Care Plans

ADMISSIONS:

  • Obtains referrals from hospital social workers, physicians, or family members keeping wait lists current for healthcare facility.
  • Functions as the Health Center public relations person, providing tours of the facility and pertinent information regarding admissions including visits to hospital discharge planners and doctor’s offices.
  • Conducts admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information and medical information, attending physician information, and other pertinent and required documentation prior to admission.

KNOWLEDGE AND SKILL REQUIREMENT

1. Bachelor's degree in Social Services or related field, LSW required.

2. 3-5 years of experience working with geriatric patients.

3. Knowledge of Federal and State regulations for skilled nursing facilities.

4. Comprehensive understanding of the English Language; must be able to read, write and communicate in English.
