Palliative Care NP Opening in Indiana

This opportunity will allow APP to provide care to the physical, functional, psychological, and spiritual needs of individuals presenting with a serious illness.


Focusing on a person-family-centered approach to care by providing symptom relief from the stress and burden of a disease process.


Health center is located in east central Indiana with all the advantages of a single hospital community, low managed care environment, great working environment and excellent medical staff.


Position Details:

  • Performs history and physical examinations on patients.
  • Dictates appropriate hospital reports in a timely manner
  • Establishes presumptive diagnoses, establishes the general work up of the patient by ordering appropriate laboratory studies and is responsible under the physicians supervision for the management of the patients problems following diagnosis.
  • Initiates appropriate laboratory, radiological and special examinations or test required for the evaluation.
  • Provides counseling and instruction regarding patient problems, referring patients to other services.
  • Provides follow up care to patients per physicians plan of care
  • Coordinates care with discharge planners: Nursing Homes, etc.

Qualifications:

  • Experience with end of life and chronic illness
  • Compassion for ill patients
  • Great communication skills
  • Licensure: Indiana Medical License
  • Certifications: Must maintain BLS/ACLS
