Opportunity Overview:

The Department of Veteran Affairs is seeking a qualified Geriatrics and Extended Care Physician. This role encompasses a variety of services, including outpatient geriatrics, nursing home care, home-based primary care, inpatient and outpatient palliative care, hospice services, pre-operative optimization, transitional care, and partnerships with State Veteran Homes. The position also involves working with Geriatrics and Hospice and Palliative Medicine fellows, Internal Medicine residents, and medical students.

Key Responsibilities:

  • Lead interdisciplinary teams to ensure high-quality patient care.
  • Provide primary care and consultative services in geriatrics.
  • Deliver hospice and palliative care services across the healthcare system.
  • Collaborate effectively with all referral sources.
  • Maintain compliance with patient care quality standards.
  • Act as a preceptor for medical trainees at various educational levels.
  • Deliver didactic education to trainees.
  • Develop and enhance clinic-based geriatrics and/or hospice programs.
  • Participate in quality improvement initiatives.
  • Ensure timely and accurate completion of medical documentation.
  • Provide cross-coverage as needed within GEC programs.
  • Share on-call responsibilities.

Qualifications Required:

  • Board certification in Internal Medicine or Family Medicine.
  • Board certification or eligibility in Geriatrics and/or Hospice and Palliative Medicine.

Preferred Experience:

  • Experience in Geriatrics primary care and consultation.
  • Background in hospice and palliative care services.
  • Experience in nursing home care.
  • Experience as a clinician educator in Geriatrics or HPM.
