You will be responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. Demonstrate proficient practice and decision-making, deliberate planning, and critical thinking skills.

Possess intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines.

Use nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care in Veterans Affairs (VA) and care in the community.

Serve as a liaison to internal specialty providers and collaborates with community care services to manage the care of the Veteran throughout the consult process.

Support the medical center's mission to improve timely access to care, empower Veterans to make more informed care decisions and ensure eligible Veterans who want to receive care in the community are referred.

You will triage incoming referrals, determines available care options, provide scheduling guidance, and be a resource within the service.


As the Nurse Manager (NM) you are the designated first-line supervisory authority and are responsible and accountable for the operation of their assigned units and provides coverage for other NMs as needed.

Responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance.

Function as a role model, educator, practitioner, and supervisor providing leadership to the team.

Influence care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence-based cutting-edge programs/processes.

Responsible for addressing performance and/or conduct issues, conducts investigations, collaborates with Employee Labor Relations, Union partners, and other offices to address concerns/allegations.

You will engage in professional organizations to enhance leadership and career growth. Adhere to ethical principles in decision-making, fostering accountability at personal, organizational, and staff levels.

You will equip staff with tools for culturally competent care and promote a culture of accountability for high-quality, cost-effective outcomes.

Cultivate alliances across the care continuum, building trust through active listening, engagement, and emotional intelligence.

Contribute to the development of care delivery models and environmental design, ensuring evidence-based, accessible, affordable, and equitable patient care. Ensure compliance with accreditation and regulatory standards. Align nursing/clinical objectives with organizational strategic outcomes, participating in strategy planning and execution. Promote an inclusive workplace valuing diversity to achieve the organizations vision and mission.

Maintain knowledge and ensure compliance with all regulatory and accreditation standards, Directives, policies, and standard operating procedures (SOP). Provide peers with informal constructive feedback for improvement. Support colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Foster a safe and supportive environment conducive to the professional development of healthcare professionals. Contribute professional nursing perspective in discussions with the interdisciplinary team. Partner with others to effect change and produce optimal outcomes. Evaluate outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational
level within a facility.

Pay:
Competitive salary, regular salary increases, potential for performance awards
