Participates in orientation of new patients to Medical Specialty Clinics and contributes to the development of and updates to their plan of care in consideration of patient status and policy changes.


  • Perform vitals and clinical reminders as appropriate for scheduled and unscheduled patients
  • Perform EKGs for emergent/urgent patients or preoperative patients
  • Completes clinical documentation in the electronic medical record
  • Assist in the data collection (including physical, psychosocial, emotional, spiritual, cultural and educational) for patients about to be and currently undergoing Medical Specialty Clinic treatments.
  • Notifies the clinic manager of problems and assists in problem resolution efforts as requested.
  • Participates in the education of patients regarding telehealth opportunities and facilitates that process for those patients interested in telehealth including but not limited to VA Video Connect (VVC).
  • Prepares and administers prescribed medications (oral, topical, subcutaneous, and/or, intramuscular) and performs treatments according to established policies/procedures; Observes for physical and/or emotional changes in patient's health condition, including responses to prescribed medications/treatments, and promptly and accurately documents identified changes, and reporting deviations from normal to the team RN, NP or MD.
  • Accurately and consistently identifies, documents and reports common veteran changes in condition
  • Has competence of encouraging and communicating with common challenging psychological and physical behaviors to promote optimal functioning.
  • Demonstrates the ability to work to the capacity of his/her scope of practice and communicates effectively with the veteran, families, caregivers, professionals and other supportive staff.
  • Maintain, order and refresh stock and supplies required for Medical Specialty Clinics.
  • Participates in the provision of care in the Medical Specialty Clinics at the Loma Linda VA Medical Center, and Ambulatory Care Center, as appropriate and within the LVN/LPN Scope of Practice.
  • Collaborate with Pharmacy services for medication supply.
  • Screen patients in clinic and via telephone calls.
  • Ability in accurately performing Accucheks
  • Encounter nurse clinic visits.
  • Recognizes potential urgent or emergent patient care situations, and able to rapidly call a "Rapid Response Treatment (RRT)" or "Code Blue" emergency.
  • Recognizes signs of potential or active disruptive behavior and quickly intervenes with appropriate therapeutic interventions, including notifying a provider to rapidly call the BHIP provider oncall via pager.
  • Demonstrates ability to use "Safe Patient Handling" equipment and techniques, able to transport, turn and move patients using proper equipment.
  • Recognizes and appropriately responds to breakage/malfunction of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.
  • Provides Situation, Background, Assessment and Recommendation (SBAR) report to team member
  • Clean exam and procedure rooms in between and after patient usage.
  • Check for expired supplies and medications
  • Provide input for annual selfassessment
  • All clinic daily opening duties:
Ensure rooms are open and verify clean status of rooms
