Job Summary:

The Patient Navigator Licensed Vocational Nurse (LVN) coordinates access to healthcare services for patients from initial medical diagnosis or point of referral. This role requires strong communication and organizational skills to ensure seamless patient care.

Key Responsibilities:
  • Coordinate patient access to healthcare services, including scheduling appointments and managing referrals.
  • Gather and review patient medical and insurance information to ensure accurate and efficient care.
  • Act as a liaison between patients, families, healthcare providers, and other support services to ensure effective communication and care coordination.
  • Perform patient-oriented procedures and tasks within the LVN scope of practice, education, and training.
  • Manage durable medical equipment (DME) supplies and perform wound care functions as needed.
Requirements:
  • High School Diploma or GED required.
  • Graduate from a school for Licensed Vocational Nurse (LVN) required.
  • Minimum one year experience in a medical front office setting and/or a hospital-based call center that schedules appointments for multiple clinics or departments required.
  • Minimum one year of experience working with Oncology patients preferred.
Preferred Qualifications:
  • Knowledge of medical terminology.
  • Ability to read, write legibly, and speak in English (and Spanish preferred) with professional quality.
  • Proficiency in using computer and software programs necessary to the position.
  • Ability to troubleshoot and calibrate patient care equipment.
Licensures and Certifications:
  • Active California Licensed Vocational Nurse (LVN) license required.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) required.
