Advise IDT and manager of participant-specific transportation issues.
Report changes as directed in participants' baseline status to appropriate staff. Stay involved in the development and implementation of QAPI activities.
Develop, update and maintain transportation routes using our Samsara Routing system
Dispatch drivers; coordinate internal and vendor transportation for all participant appointments. Maintain locator maps for all participants.
Prepare monthly transportation report.
Ensure coordination of vehicle maintenance and repairs.
Under the supervision of the Director, Transportation is responsible for the direct transportation of physically limited passengers to and from their destinations. Position required constant interaction with caregivers and community agencies while conducting business with Hope PACE.

Education and Experience

Transportation Dispatch experience.

Knowledge of Transportation Routing Systems

High school diploma or general education degree (GED). At least one year experience in transporting handicapped individuals and working with the frail elderly.

Certificates, Licenses, Registrations

Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers.

Reasoning Ability

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to communicate, concentrate and document effectively and accurately.

Position Requirements (not all inclusive)

Follow established scheduled, adjust schedules to met unanticipated changes, determine the safest and most efficient pick up and delivery routes.

Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.

Must be up to date on all immunizations.
