Provides direct and indirect patient care and collaborates with primary care providers as well as specialty care providers.

Collaborates with providers in Oncology, Cardiology, Pulmonology, Mental Health, and other services to address the discipline-specific, health related challenges encountered in frail, chronically ill veterans.

Recognizes complex situations that impact patient care and intervenes, using sound judgment, professional attitude, and appropriate channels.
Educates primary care teams about palliative care, available services, disease 2 process and identification, and available treatments.

Receives and responds to phone calls, voicemails, and virtual communication for Veterans, informing patients and families about lab/test results, necessary medication changes, follow-up testing, and other notifications.

Facilitates medication renewal after discussion with the prescribing provider.

Tracks and manages palliative care patients on opioids including checking OARRS report, STORM notes, controlled substance consents and ensuring the completion of urine tox screens.

Organizes and documents notes from weekly Palliative Care Interdisciplinary Team meetings.
Collaborates with Palliative Care and support teams to help address bereavement needs for families of palliative care patients.
Serves as patient advocate and assists in maintaining quality care for veterans.
Participates in team performance improvement and sustainment activities.
Collaborates with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support care delivery.
