Job Title: Advanced Practice Provider Inpatient Palliative Care

About the Role:

We are seeking an experienced Advanced Practice Provider to join our Palliative Care team at University Hospitals Cleveland Medical Center. The APP will work clinically with non-oncology patients, primarily on the inpatient consultation team. The schedule will be part-time, two 10-hour shifts per week. The ideal candidate will have experience in palliative care, geriatric, or hospice care.

Key Responsibilities:

  • Conduct thorough patient assessments and develop individualized care plans.
  • Collaborate with other healthcare providers to provide comprehensive care to patients and families.
  • Order and perform routine diagnostic procedures as needed.
  • Develop and implement treatment plans in accordance with established protocols and standards of care.
  • Monitor the effectiveness of therapeutic interventions and make adjustments as necessary.
  • Contribute to the educational development of staff, patients, and families.
  • Provide education and support to patients and families through role modeling and in-services.
  • Stay up-to-date with the latest research and best practices in palliative care.

Requirements:

  • Demonstrated competence as a practitioner.
  • Effective oral and written communication skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Ability to maintain confidentiality and handle sensitive information.

Education/Expertise:

Nurse Practitioner: Nursing Doctorate or Master of Science in Nursing with emphasis on clinical practice.

Physician Assistant: Completion of a formal Physician Assistant training from an accredited program with NCCPA certification.
