Responsible for knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day-to-day operation of the program, and all reporting requirements within the assigned Veterans Integrated Systems Network (VISN).

Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex patient care.

Provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling.

Possesses thorough knowledge of PCAFC, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines.

Accountable for utilizing critical thinking skills and sound judgment to ensure caregivers are provided with a thorough, comprehensive review when appropriate.

Demonstrates knowledge and ability to apply principles of the CSP and CEAT practice.

Applies the nursing process and care coordination principles within a collaborative, interdisciplinary practice setting, which will then enable effective application of CSP practices and principles.

Core functions include thorough assessment of patient care needs to determine the most appropriate level of program participation.

Uses knowledge of the CSP to matrix Veteran level of functioning with the appropriate CSP services (i.e., Program of General Caregiver Support Services [PGCSS]; PCAFC; and/or Veteran entitlements and benefits) in order to ensure patient safety, recovery, and program stewardship.

Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise.
Coordinates within the CSP to develop and implement strategies for performance improvement and enhancement of the program.
Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
Co-facilitates educational groups and trainings for program participants in support of Caregiver and Veteran health and wellness.
Completes routine wellness visits with Caregivers and Veterans at home.

Provides supportive care coordination to a panel of Caregivers and Veteran, and assesses Caregiver wellness and self-care needs to determine the most appropriate resources, referrals and CSP programming.

Pay:
