Licensed Vocational Nurse Manager Job Description

Purpose:

To assist and ensure that resident care is provided in a safe and effective manner, while upholding the principles of resident rights, compliance with state regulations, and in accordance with currently recognized nursing care standards and practices for elder care. Leads daily clinical and administrative functions collaborating with physicians, therapists, and other healthcare providers to develop and implement individualized care plans for residents. Serves as a mentor and leader, fostering a positive work environment while providing guidance and support to the community. Works closely with families, addressing their concerns and providing updates on the well-being of the residents.

Minimum Eligibility Requirements:

Licensed Nurse in good standing with State Licensing Authority.

Experience in training and team member development preferred.

Minimum of 2 years experience working in the field of dementia care or the coexistence of two or more chronic diseases in an individual.

Ability to establish effective relationships with residents, family members and team members.

Must be able to read, comprehend, write, and speak English to communicate effectively with residents, families, other team members and physicians.

Must be able to communicate effectively with residents, families, team members, community and state officials, referral sources and the general public.

Strong documentation skills.

Must have basic computer skills, including Word and Excel.

Ability to develop systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident care.

Strong organizational skills and ability to re-prioritize daily tasks, in order to accommodate fluctuating needs of the residents and the community.

Ability to represent the community in a positive and professional manner.

Must be able to meet all health-related requirements pertaining to state licensure.

Must maintain current CPR/First Aid credentials.

Must maintain current Food Handlers credentials.

Must maintain annual Continuing Education Credits.

Must be able to perform duties and responsibilities (essential functions) with or without accommodation.

Essential Functions:

Management

Works with the Executive Director and Health & Wellness Director to implement the principles and processes for providing resident care and personal services. This includes resident care needs assessment, meeting quality standards for services, and evaluation of resident and family satisfaction.

Assists the Executive Director and Health & Wellness Director to develop and implement training on human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.

As a member of the Leadership Team will work to identify complex problems and review related information to develop and evaluate options and implement solutions.

Works with the Department Managers and other community leaders to provide training to all community team members on dementia-related topics and assists in maintaining team member training records.

Provides supervision for care team members including assisting with the hiring, training, evaluating, and providing progressive discipline.

Serves as a role model to other team members in demonstration of courtesy, gentleness, effective approach, and interaction with residents.

Provide mentoring and education to care giving team members to ensure proper care and follow through of all care needs.

Assist with team member performance concerns and/or guidance through mentoring.

Ensures an adequate inventory of healthcare supplies and equipment and follows appropriate community protocol for purchasing.

Maintains knowledge of current OSHA and state regulations, and routinely monitors Community compliance with the regulations related to resident care and nursing functions.

Recruitment & Scheduling

Leads the recruitment process for the care team. This includes participating in candidate selection, conducting interviews, and assessing candidates' qualifications, skills, and compatibility with the community's care philosophy.

Employs various recruitment strategies to attract a pool of qualified candidates. This may include posting job advertisements at local nursing schools, professional associations, and community organizations to generate interest and referrals.

Leads the final decision-making process to select the candidates who will be offered positions considering input from the interviewers and aligning with community's policies and procedures, standards, and values.

Aids with the onboarding and orientation process of the care team. This includes facilitating the completion of necessary paperwork, coordinating required training programs, and ensuring that new hires are familiarized with the community's policies, procedures, and culture.

Actively focus on recruiting and retaining quality care team members, contribute to the stability and effectiveness of the care program by building strong relationships between the care team, residents, and families to strengthen the effectiveness of the care program.

Manages and coordinates the scheduling of the care team to ensure adequate coverage and appropriate skill mix. This involves creating staff schedules, considering resident needs, optimizing staff availability, and managing time-off requests. Flexibility and responsiveness to unexpected staffing changes or emergencies are crucial in this role.

Develop and post a work schedule to ensure adequate staffing on a daily basis.

Ensure timely and accurate input of staff work schedule (including all required training and meetings) into online scheduling system.

Main point of contact and trainer for care team and care management regarding online staff scheduling system

Communicate with care team about trainings, meetings, in services, etc.

Resident Care

Assist with resident assessments and reassessments, development, and implementation of care plans. Includes the resident and family in planning care, establishing priorities, and setting goals.

Reviews and develops care plans by using assessment information to develop a care plan before the end of duty time that communicates enough information for incoming staff to adequately care for the resident.

Ensures that changes in resident condition are monitored, evaluated, clearly and concisely documented including documenting the resident response and outcome, following through ensuring effective interventions are implemented.

Identifies variations in expected vital signs and lab tests; documents, reports and follows through on abnormal findings with physicians and the Health & Wellness Director and /or the Executive Director.

Works in partnership with the Health & Wellness Director and/or the Executive Director to conduct resident and family care conferences.

Assist with communication between residents, family, and physician regarding resident health care issues.

Performs skilled nursing tasks as defined by Community policy and procedure, and as allowed by state law.

Provides support and assistance to medication management systems and ensures that medications are dispensed in a safe and effective manner, that MARS are accurate and complete, and that medication cart is orderly, with all medications appropriately packaged and stored.

Ensures effective monitoring of resident health concerns such as weight and nutrition, skin integrity, mobility, fall management, and pain management.

Reviews Resident Health Record on a routine basis and ensures that documentation meets Community standards and adequately addresses the residents healthcare issues, including identified concerns, interventions, and outcomes.

Ensures that all resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, and respect

Give clinical direction and guidance to care giving team members.

Other

Attends all required team member meetings and in-service training sessions.

Upholds Residents Rights as defined by state regulations and by Company policy, at all times.

Supports and upholds all other Company policies, and the service philosophy.

Reliable and predictable attendance.
