Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

Duties


This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30th, 2024. The initial cut-off date for referral of eligible applications will be December 29th, with subsequent cut-off dates every 2 weeks. The physician serves as the Primary Care Physician (PCP) Director for the Community Based Outpatient Clinic.

The physician will have both administrative duties and direct patient care duties with 30% of his or her time devoted to administrative duties as CBOC Medical Director and 70% devoted to direct patient care.


The Medical Director leads provider teams at this site and report to the Director of Primary Care for Medical Service and provides comprehensive care for our Veteran population.

Supervisory functions include (but not limited to) supervision of providers, ensure adequate staffing, leave approval, ensure and guide the clinic staff/providers to meet various clinical performance measures and access requirements, assist the Director Primary Care for Medical Service and facility leadership meet various VHA requirements, assist in meeting various regulatory agencies' (like OIG, JC and others) standards.

Helps address customer care related issues of the PC clinics.

The incumbent will also lead the PACT team in coordinating care of the veterans in non-face to face or group settings with team members.

Responsible for monitoring and giving feedback on MD productivity data. Involved in at least one local QI project per year.

The CBOC operates under a Patient Aligned Care Team Model.

Together with his or her teamlet, comprised of a PCP, RN, LVN, and a Clerk, the physician cares for a panel of approximately 1,000 patients.

The physician spends 70% of his or her time as the PCP for the teamlet. The physician provides primary care services in a team based setting, including chronic disease management and preventive health. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain.

The incumbent will also lead the PACT team in coordinating care of the veterans in non-face to face or group settings with team members.
