Job Summary

The Nuestro Case Manager is responsible for assisting Highwood community with finding and coordinating resources and benefits. Case Managers will also work with expressive therapists to assist with programing and community outreach. Case Managers act as an advocate for their client's health and make sure they receive the right care for their situation. The Case Manager works with healthcare professionals like Doctors and Nurses to come up with a treatment plan that best fits their client's needs. The Case Manager also is responsible for duties assigned by the Clinical Operations Manager. Case Managers arrange transitional plans for clients to continue receiving excellent care after being discharged from the hospital/mental health center. Transitional plans ensure
that clients continue to improve even without the constant care provided in a community mental health setting.

Essential Responsibilities

Primary Responsibilities:

  • Provide face-to-face mental health annual assessments if needed, including updating paperwork necessary to meet state health standards.
  • Provide support with community organizations when needed
  • Facilitate client referrals for additional services consistent with assessed needs physician recommendation and/or client desired outcomes
  • Provide brief crisis intervention, stabilization services and supportive counseling
  • Refer client to Program Director when appropriate
  • Regularly consult with expressive therapist and other case managers
  • Provide back-up coverage for other case managers and or expressive therapist
  • Other duties as assigned by Program Director
  • Assumes leadership role either by own initiative or when asked by Program Director
  • Receive input from the Program Director/Clinical Coordinator and provide feedback regarding administrative issues
  • Participate in community events
  • Participate in supervision with Program Director
  • Participate in Staff Development seminars
  • Attend weekly case management meetings
  • Clear vacation and time off request with Program Director
  • Attend weekly case presentation meetings when possible
  • Appropriate and timely response to Program Director requests for data or special projects

Administrative Duties and Metrics:

  • Complete all clinical documentation and required reports in an accurate and timely manner, including treatment plans(every 6 months), service tickets and progress notes (withing 3 days), LOCUS
  • Complete referral, transfer, and discharge paperwork in timely manner: referral and transfer, immediately upon need; discharge summary and instructions within three working days of termination
  • Complete annual mental health assessments and accompanying paperwork timely and update CIS
  • Complete Treatment Plan, referral transfer and discharge paperwork and annual mental health assessment accurately
  • Participate in orientations and training programs of the Center
  • Protect client confidentiality and abide by the ethical statement of the agency
  • Maintain Productivity goals as determined by supervisor

Qualifications

  • Licensed for independent clinical practice in the State of Illinois in mental health or related field
  • Minimum of 3-5 years of progressive and successful clinical and crisis experience preferable in an outpatient mental health setting
  • Demonstrate competence in interdisciplinary and inter-agency collaboration
  • Minimum of 3-5 years of progressive and successful clinical supervision experience preferable in an outpatient mental health setting
  • Demonstrated administrative, supervisory, and organizational skills
  • Advanced oral and written communication skills
  • Ability to think critically and analytically and to problem solve in both short and long term contexts
  • Working knowledge of best practices in clinical counseling in the field of mental health
