Role Responsibilities
The Registered Nurse Case Manager collaborates with healthcare providers, patients, and their families to ensure optimal health outcomes for individuals with complex medical conditions.

The Case Manager's duties include enhancing the effective use of healthcare services, identifying chronic or high-risk cases, and initiating comprehensive case management in line with established guidelines.

Responsibilities also encompass managing patient care across various healthcare settings to ensure appropriate levels of care, preventing unnecessary hospital readmissions, and addressing the medical, environmental, and psychosocial needs of patients throughout their care journey.

The Nurse Case Manager serves as an advocate, facilitating communication and coordination among the healthcare team and families regarding care planning and recovery objectives.

Furthermore, the Case Manager will evaluate and coordinate available resources for patients, optimizing the utilization of healthcare benefits.

Key Responsibilities Include:

  • Analyzing data from predictive modeling tools to identify patients eligible for the Case Management Program.
  • Enrolling and managing a caseload of patients with complex medical needs, ensuring compliance with health plan requirements.
  • Developing tailored case management plans with prioritized goals and appropriate interventions, adhering to risk stratification requirements.
  • Maintaining proficiency in current Case Management Standards.
  • Coordinating healthcare services with relevant physicians and providers.
  • Compiling comprehensive summaries and logs of all active Case Management cases as required.
  • Serving as a resource for Utilization Management staff, physicians, and patients.
  • Assisting in the evaluation and revision of Case Management policies and procedures.
  • Participating in continuing education and training relevant to job functions.
  • Performing additional duties as assigned.
Qualifications

Licensure/Certification:
  • Must possess an active Registered Nurse License in good standing.
  • Certification in Case Management is preferred.
  • Current CPR certification (BLS) is required.

Education:
  • An Associate's degree in nursing is required; a Bachelor's degree in nursing is preferred.

Experience:
  • 2-4 years of experience in a primary care or hospital setting as a Registered Nurse.
  • Previous experience in case management is preferred.
