Case Manager- Intensive Program

Hope For Youth is looking for a dedicated Intensive Case Manager to join their team and work in the Intensive & Supportive Case Management (ISCM) program. This program aims to provide essential support and treatment to children and families, helping to stabilize the immediate living situation of youth and prevent them from entering foster care or higher levels of care.

The Intensive Case Manager will be responsible for:

  • Delivering services to maintain children in their current living environment and working towards positive outcomes as outlined in the service contract/grant
  • Assessing and addressing safety and health concerns in the child's home environment and advocating as necessary
  • Conducting regular face-to-face and community contacts with clients and completing all required reports and paperwork
  • Demonstrating flexibility in adapting to changes in the program model and job responsibilities
  • Managing a caseload of up to 4 clients

In this role, you will spend your time engaging in a variety of tasks, including fieldwork, documentation, attending meetings and training sessions, and handling any other assignments as needed.

Minimum Requirements:

  • BSW or equivalent degree in a related field
  • Valid NYS Driver's License and access to a reliable vehicle for home visits
  • Strong writing skills and time management abilities
  • Flexibility with work hours, including evenings and weekends if required
  • Availability for around-the-clock communication and emergencies
  • Physical capabilities for various job requirements

This position offers a salary of $45,000 per year for 35 hours per week, providing a range of benefits and support for employees. Some of the benefits of working at Hope For Youth include:
