Our mission is to transform the lives of youth involved with the child welfare and juvenile justice systems, as well as those experiencing homelessness. Our team of multi-disciplinary professionals is available 24/7 to ensure that we meet the highest standards of care and compliance.


Position Summary:

The Aftercare Caseworker plays a crucial role in facilitating aftercare services from the moment a youth is placed in our program until their successful discharge. This position is essential for ensuring a smooth transition back into the community. The Aftercare Caseworker collaborates closely with case managers and program managers to provide comprehensive support to youth throughout Nassau and Suffolk County.


Key Responsibilities:

  • Adhere to program policies and procedures as outlined in the practice guide and employee manual.
  • Complete all necessary reports and maintain accurate case documentation.
  • Coordinate with the Department of Social Services, educational institutions, and families/legal guardians.
  • Provide crisis intervention and counseling as required.
  • Ensure compliance with state regulations and uphold a professional code of conduct.
  • Develop and implement discharge plans, follow treatment protocols, and make referrals as necessary.
  • Conduct home visits across Suffolk and Nassau County.

Daily Activities:

  • 60% dedicated to general casework responsibilities, including documentation and reporting.
  • 30% focused on supporting youth and their families.
  • 10% allocated for staff meetings and training sessions.

Qualifications:

  • Bachelor's degree required; Master's in Social Work preferred.
  • Minimum of one year of relevant social work experience.
  • Valid NYS driver's license in good standing.
  • Physical requirements include the ability to operate a vehicle, lift/move up to 50 pounds, and engage in various physical activities as needed.

Compensation and Benefits:

This part-time position offers an hourly wage of $20.00-$22.00, with a flexible schedule allowing for 16-24 hours of work per week.
