About Hope For Youth
Hope For Youth's Residential Services Department is dedicated to providing community-based, trauma-informed, and family-centered support to adolescents and young adults across Nassau and Suffolk County. We serve youth entering through various systems, including child welfare, juvenile justice, and those experiencing homelessness. Our multi-disciplinary team of professionals is available 24/7 to ensure that we meet all accreditation and licensing standards.
Role Overview
The Aftercare Caseworker will be instrumental in delivering aftercare services from the initial day of a youth's placement until the conclusion of voluntary aftercare services. This role is crucial for facilitating a successful transition back into the community post-placement. The Aftercare Caseworker will collaborate closely with case managers and program managers to provide comprehensive support to youth throughout Nassau and Suffolk County.
Key Responsibilities
- Adhere to the policies and procedures outlined in the program practice guide and employee manual.
- Complete essential reports and maintain accurate case documentation.
- Coordinate with the Department of Social Services, educational institutions, parents/legal guardians, and other relevant contacts.
- Provide crisis intervention and counseling as required.
- Comply with state regulations and uphold the applicable code of conduct.
- Develop and implement discharge plans, follow treatment plans, and submit referrals as necessary.
- Conduct home visits throughout the designated counties.
Daily Activities
- 60% dedicated to general casework responsibilities, documentation, and reporting.
- 30% focused on supporting youth and their families.
- 10% participating in staff meetings and training sessions.
Qualifications
- Bachelor's degree required; Master's in Social Work preferred.
- Minimum of one year of relevant social work experience.
- Valid NYS driver's license in good standing.
- Physical requirements include operating a vehicle with up to ten youth, lifting/moving up to fifty pounds, and performing various physical tasks as needed.
Compensation and Benefits
This part-time position offers an hourly wage ranging from $20.00 to $22.00, with a flexible schedule allowing for 16-24 hours of work per week.
