Geriatric Care Coordinator

We are currently looking for a per-diem Geriatric Care Coordinator to evaluate and address the personal care and healthcare needs of our residents. The ideal candidate will oversee the complete patient care process. Responsibilities include assessing, planning, implementing, monitoring, and evaluating the necessary actions to fulfill the health and human services required by clients. The incumbent will also ensure compliance with all relevant regulations, including privacy laws, while collaborating effectively within a team.
Applicants should possess a Bachelor's degree from an accredited institution in nursing, social work, or a related field, along with a minimum of 2-4 years of experience working with the elderly population. Certification in First Aid/AED is required, and having an LPN qualification is advantageous.
All interactions among the Geriatric Care Coordination team and residents will reflect the core values of DOMINICAN VILLAGE.
Key Responsibilities:
- Engage in respectful interactions with residents, ensuring their dignity and independence are prioritized, while fostering a quality living environment.
- Participate in regular meetings with the Director of Social Services to review cases and refine objectives aimed at supporting residents in maintaining their autonomy.
- Conduct resident assessments and develop service plans; monitor the effectiveness of programs in meeting residents' needs; provide necessary information for the Resident Status committee. Ensure timely completion of required documentation.
- Guide and support new residents in adapting to daily routines.
- Update and audit EMERGENCY records periodically to ensure accuracy.
- Respond to emergencies and ensure thorough follow-up, including accurate documentation of unusual occurrences and progress notes.
- Coordinate with hospitals, rehabilitation centers, and social services to facilitate resident discharge processes and ensure appropriate home care services.
- Identify and report any potential risks or incidents that could impact the safety and well-being of residents, staff, and visitors.
- Bachelor's degree in social sciences, social work, nursing, or a related discipline.
- At least 2 years of experience in public health or a related field.
- A minimum of 1 year of current experience in case management.
- Strong organizational, time-management, and follow-up abilities.
- Excellent written and verbal communication skills, demonstrating professionalism.
- Proficiency in Microsoft Office Suite (Excel, PowerPoint, Word, and Outlook) and familiarity with CRM software is preferred.
- Experience in home care or assisted living settings is beneficial.
Work Schedule:
- Per diem.
