JOB SUMMARY:

The CARES Prevention Coordinator works under the direction of the CARES Program Manager and with the CARES team to provide guidance, counseling, and advocacy to clients in need of assistance accessing Pre-Exposure Prophylaxis (PrEP), Non-Occupational Post Exposure Prophylaxis (PEP), Sexually Transmitted Infections (STI) testing and other vulnerable populations at high risk for HIV transmission. The CARES Prevention Coordinator will provide high-intensity care coordination, patient navigation, and accompaniment, as well as management of a PrEP patient database for panel management and quality improvement initiatives.

PRIMARY DUTIES AND RESPONSBILITIES:

PrEP care coordination:

  • Serve as an on-site resource to counsel patients, and support medical providers in initiating PrEP, including ordering appropriate labs and following health center workflows.
  • Perform and document sexual health risk assessments on patients who have tested positive for one or more STI's. Counsel patients on ways to reduce STI risk, including barrier methods and PrEP.
  • Conduct outreach in a variety of settings (treatment programs, community events, etc.) to educate patients about PrEP services. May serve as first contact for patients reaching out to the health center in response to advertising or educational materials.
  • Assist patients in addressing insurance barriers, referrals for benefits, and transportation; provide reminder calls for visits and proactively contact patients to remind them of appointments and lab work.
  • Connect with patients at their place of residence, as needed, to accompany them to appointments, reconnect them with care, and/or provide medical case management.

PrEP medication adherence support.

  • Provide adherence counseling, coordinate PrEP refills with pharmacy staff, and assist with medication delivery and storage.

Social service coordination:

  • Assist clients with identified health-related social service needs, help them access related resources, and support the successful utilization of these resources.

Drug use health promotion and harm reduction:

  • Provide patients with health promotion, behavioral risk reduction, and prevention of communicable disease acquisition or transmission by offering health education and conducting educational groups in a variety of outreach settings. Duties may include providing and counseling patients on use of naloxone for opioid overdose reversal, counseling on ways to reduce risk from injecting drugs, and referral to needle exchange services.

Quality improvement and PrEP program database management:

  • Maintains concise and accurate documentation according to departmental standards including but not limited to chart notes in patient electronic medical record.
  • Manage a database of patients who have been prescribed PrEP. Track data on patient touches for reporting purposes and assist in report preparation.
  • Participate in research activities related to PrEP program activities as indicated.

HIV Counseling and testing:

  • Provide HIV counseling and order testing for HIV, HCV, HBV, and syphilis serologies and other PrEP intake and maintenance testing, including counseling patients on self-collection of extragenital STI test samples.
  • Link patients with positive test results to appropriate clinical services.

Other Duties:

  • Educate and distribute culturally sensitive educational materials.
  • Maintain competency in areas of responsibility to ensure ongoing provision of high-quality care to patients.
  • Employ effective verbal, written, and interpersonal communication skills to facilitate interactions with various members of the health care team and patients.
  • Establish working relationships with patients, other health care professionals, and the greater Mattapan community.
  • Demonstrate sensitivity during interactions with patients and colleagues with respect to ethnic, cultural, gender and sexual orientation. Exhibit a commitment to diversity and inclusion within the workplace.
  • Adhere to medical-legal protocols regarding management of confidential or sensitive information.
  • Cooperate fully with MCHC's Corporate Compliance Program
Requirements:

REQUIREMENTS & QUALIFICATIONS

Requirements:

  • Knowledge and experience with HIV services including HIV education/prevention, outreach, counseling, emotional/psychological support and or care management.
  • Experience providing HIV or substance abuse education.
  • Understanding of Harm Reduction approaches
  • Experience working with diverse and/or special populations, including but not limited to Black or African community, Latinx community, LGBTQIA+, persons who inject drugs, immigrants.
  • Ability to work independently and in small groups.
  • Facility with database management

Preferred Qualifications:

  • Bachelor's degree in science with one year of related work experience or 3-5 years of relevant experience in a related health care field
  • Knowledge of HIV prevention strategies
  • Experience providing HIV or substance abuse education preferred but not required.
  • Strong interpersonal and communication skills
  • Experience working in an ethnically, culturally, and racially diverse environment.
  • Ability to meet deadlines and work within a team-based environment.
  • Experience with electronica data collection and databases (Access, Tableau)
  • Bilingual in Haitian-Creole or Spanish preferred, or comfort utilizing medical interpreter services.
