Position Summary: The Community Outreach Coordinator at Tidelands Health is tasked with providing and implementing community outreach services, organizing community classes, health fairs, events, and outreach activities for the hospital system. This role involves supervising a team that delivers outreach and education services within the department. Ideal candidates should have experience in staff supervision, grant management, program development, and outreach initiatives.

The responsibilities of the Community Outreach Coordinator include:

  • Community Outreach and Engagement
  • Ensuring regular provision of outreach services and maintaining a monthly calendar of events and classes for the community
  • Providing community classes and outreach services aligned with community needs and the Community Health Needs Assessment (CHNA)
  • Marketing, organizing, and conducting community outreach events in various locations within the Tidelands Health service area
  • Collaborating with community partners and organizations to host events and maintain community engagement and partnerships
  • Grant and Financial Management
  • Assisting in budgeting for grant programs related to outreach services
  • Drafting grant applications, progress reports, and ensuring compliance with grant requirements
  • Maintaining documentation and metric measurements for grant reporting and impact analysis
  • Supervision and Business Operations
  • Providing leadership, supervision, and direction to the community outreach unit aligned with organizational objectives
  • Researching and proposing health improvement programs for the community
  • Managing human resources within the community wellness unit including recruitment, training, and staff retention
  • Reporting ongoing community wellness activities for internal and external communications
  • Ensuring compliance with hospital and departmental policies
  • Providing coverage and support for the community wellness supervisor

QUALIFICATIONS

  • Three or more years of experience in community outreach, health education, social work, or nonprofit management
  • Experience in healthcare education, community coordination, and grant management is preferred
  • Proficiency in grant writing, management, and reporting is desirable

Education:

  • Bachelor's degree in nursing, health education, social work, nonprofit management, or related field

Knowledge/Skills:

  • Understanding of the healthcare system, population health, and social determinants of health
  • Ability to work independently, prioritize tasks, and build relationships effectively
  • Strong interpersonal and communication skills
  • Proficient in business planning, grant management, and relationship building
  • Organizational, problem-solving, and PC skills required

Other Requirements:


• Personal vehicle for transportation and mobility across the service area


• Valid SC driver's license

Physical Requirements: Light-Medium Physical Agility Test (PAT) Rating

The role involves frequent standing, sitting, walking, manual dexterity, and communication. The employee should be able to lift up to 20 pounds occasionally and 5-10 pounds frequently.
