Overview:
Support individuals in self-directing Community Habilitation services. Provide life skills training, supervision, and ongoing assistance to help them achieve personal outcomes.
Responsibilities:
- Practice and promote agency's mission statement through family and community engagement.
- Assist individuals in reaching their personal outcomes as per Self-Directed budget and Community Habilitation plan.
- Ensure support for all daily living activities including personal hygiene, dressing, meals, and housekeeping.
- Offer companionship, supervision, and instruction while maintaining safety in both home and community.
- Document service implementation and progress through Self-Direction daily notes.
- Submit required paperwork promptly and alert supervisors of any issues affecting individuals.
- Adhere to regulations and agency policies, complete necessary trainings, and perform other job-related duties as assigned.
Qualifications:
- High school diploma or equivalent preferred.
- Prior experience working with individuals with intellectual and developmental disabilities preferred.
- Valid NYS driver's license meeting agency's insurance standards.
- Strong communication, organizational skills, and ability to follow directives.
- Team player with a positive attitude.
