Job Description:

Kaiser Permanente is currently seeking a dedicated Medical Social Worker II to join our team. As a Medical Social Worker II, your main responsibility will be to help patients and their families/caregivers deal with the social and emotional challenges and practical arrangements related to the patient's illness. Working under the guidance of the Social Work Manager/LCSW, you will provide age-appropriate social work care to patients and caregivers in line with agency policies and state/federal regulations.

Key Responsibilities:
  • Offer psychodynamic interventions, crisis intervention, grief counseling, and stress reduction.
  • Develop and implement individual treatment plans for patients and families.
  • Collaborate with healthcare team members to assess and coordinate patient care.
  • Provide information and referrals to community resources.
  • Maintain accurate documentation and records.
  • Participate in Quality Assurance activities.

Requirements:
Experience
  • Step I: 0 - 2 years of social work experience.
  • Step II: 2 - 4 years of social work experience within the last five (5) years.
  • Step III: 4 or more years of social work experience within the last ten (10) years.
Education
  • Must have a Master's in Social Work accredited by the Council of Social Work Education.
License, Certification, Registration
  • N/A

Additional Qualifications:
  • Ability to work effectively in a team setting.
  • Solid psychosocial assessment skills.
  • Strong communication skills, both oral and written.
  • Proficiency in computer systems and clinical information platforms.
  • Capacity to handle a demanding workload efficiently.
  • Experience in benefits administration and care coordination.

Preferred Qualifications:
  • LCSW preferred.
  • Prior experience in a healthcare setting.
