About the Role

We are seeking a compassionate and skilled Medical Social Worker to join our team at Alameda Care Hospice. As a Medical Social Worker - Case Manager, you will play a vital role in providing emotional support and connecting patients and their families with community resources.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop personalized care plans
  • Provide counseling and emotional support to patients and their families, including grief and loss counseling
  • Coordinate community resources and services to meet patient needs
  • Serve as an advocate for patients and their families, helping them navigate the healthcare system
  • Participate in interdisciplinary team meetings to develop and review patient care plans
Requirements
  • Master's degree in Social Work from an accredited school of social work
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
  • Minimum of 2 years of experience working in hospice or palliative care
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of an interdisciplinary team
