Job Description
  • Conduct biopsychosocial evaluations for individuals, groups, and families, focusing on emotional and mental wellness as well as substance use disorders.
  • Create culturally sensitive discharge plans that encompass the holistic aspects of care, including physical, emotional, social, and spiritual dimensions.
  • Demonstrate expertise in trauma-informed care and proficiently apply clinical practices.
  • Evaluate patient care plans continuously to assess health outcomes and actively contribute to Performance Improvement initiatives.
  • Utilize evidence-based methods to deliver effective case management services.
  • Offer relevant referrals and linkages based on patients' clinical diagnoses.
  • Independently diagnose and address psychosocial and emotional conditions using evidence-backed approaches.
  • Solicit feedback to ensure ethical practices and quality care delivery.
  • Provide consultations on clinical recommendations and educate on various health-related issues.
  • Advocate for patients and practice areas using system theory approaches.
  • Participate in healthcare system councils or committees to drive initiatives forward.
  • Review and ensure compliance of clinical documentation within the assigned practice area.
  • Accomplish all job responsibilities within the department as per supervisory guidance.
Qualifications
  • Hold a Master's degree in Social Work accredited by CSWE.
  • LC** (**ensed Certified Social Worker-Clinical) certification from the Maryland Board of Social Work Examiners.
  • Possess a minimum of two years of post-Master's experience.
