Key Responsibilities
  • Conduct comprehensive psychosocial assessments and develop individualized care plans to address the unique needs of each patient.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and ensure seamless care transitions.
  • Provide direct psychotherapy and psychosocial care to patients, including crisis intervention, grief counseling, and suspected abuse reporting.
  • Monitor the effectiveness of medication and report any side effects to physicians.
  • Develop and implement action plans to mitigate psycho-social barriers to discharge and promote patient engagement and education.
Requirements
  • Master's degree in Social Work (MSW) required.
  • Current licensure as a Licensed Master Social Worker (LMSW) in Maryland.
  • Up to one year of experience in a healthcare-related field, with field placement considered as relevant work experience.
  • Excellent interpersonal and communication skills, with the ability to work collaboratively within an integrated interdisciplinary team.
  • Comprehensive knowledge of community resources and ability to gather relevant data to inform patient care decisions.
