Key Responsibilities- Collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
- Develop and implement individualized care plans to address the unique needs of patients and their families.
- Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.
- Identify and address social determinants of health that impact patient outcomes.
- Provide education and support to patients and families on health-related topics and community resources.
- Master's in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker-Certified (LCSW-C) preferred.
- 2+ years of hospital social work or community social services experience, including post-graduate internship placement and/or related experiences.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Demonstrated ability to follow verbal instructions and communicate effectively both verbally and in writing.
