The Family Development Center at Governors State University invites qualified candidates to apply for consideration.

In partnership with parents and staff, implement a developmentally appropriate home-based program in compliance with Early Head Start Program Standards. Home Visitor is responsible for providing direct services to families to help children grow and develop in a healthy, positive way. Home Visitor provides support, training, and education to families about issues such as child development, heath and nutrition, community resources quality care giving, maternal life development, family support, case management and social service referrals.

Essential Responsibilities

In partnership with each child’s parents, integrates all aspects of the Early Head Start Performance Standards into the one and a half-hour home visit curriculum for each child, ages 0-3; including assessment information and previous plans such as IFSP’s. At minimum, forty-eight (48) visits per year to be delivered in the family’s home. Visits for prenatal participants are performed on a monthly basis for the duration of the pregnancy.Plan and implement all aspects of the Parents as Teachers Curriculum and/or Partners for a Healthy Baby Curriculum for all Home Visiting Parents and Children.Perform assessments according to assigned schedule. Use the information to design individualized lesson plans and programming to meet the developing needs of assigned children/pregnant mother. Assist parents and expectant mothers/fathers in their understanding of their child’s growth, development, and individual needs. Assure all screenings of assigned children/prenatal mother are performed within 45 days as prescribed in the EHS Performance Standards and report any findings.Collaborate and coordinate with the interdisciplinary team including but not limited to the HSD, FSD, ED, PreK FSA and EHS FSA to enroll and orient families to the FDC.Complete and maintain Family Partnership Agreements, Family Goals and Child Goals with each enrolled family. Update the goals frequently.Maintain, track and follow-up on all medical information, dentals, physicals, and immunizations for all families on caseload by assigned benchmarks according to the EPSDT and EHS regulations and requirements. Assist families to obtain and ensure all children/prenatal mothers are up to date with requirements. Document and track all referrals and follow-up.Organize and implement group socialization/developmental playgroup sessions at least twice monthly with group lesson plans for enrolled children and parents. Assure that children are supervised at all times.Abide by program philosophies on parent involvement and parent participation. Establish a trusting relationship with families by initiating and maintaining regular client contact.Actively participate in parent involvement activities. Encourage parent involvement and parent participation in a variety of ways to include but not limited to, face-to-face, telephone, video, and written contacts. Communicate with parents both verbally and in writing.

Implement the EHS transition plan by performing transition meetings of children with their families, those transitioning between programing and transitioning out of the program, coordinating with the EHS FSA and PreK FSA, assisting the family to find other community childcare, school district or Head Start programing.When necessary, report any health or social service needs of enrolled children/prenatal mothers to the appropriate Director and assist the family in finding needed resources in the community. Follow up with assigned families to assure the health, crisis or social service needs were met. Document and track all referrals and follow up.Actively plan, track, recruit, and market the FDC Programs within the community/service area.10%
Keep all children’s/prenatal mothers files in a centralized location and file all documentation regularly. Perform all required documentation and reports and submit by assigned deadline.Enter all information on assigned children/prenatal mothers and families into a computerized database and tracking system within 24 hours.Assist families needing transportation for medical, social service, play group or other non-childcare needs to appointments, including providing transportation using agency van to and from Parent meetings and Policy Council as needed.10%Comply with regulatory health requirements, including: a pre-employment physical with lift test, proof of MMR and DTAP Immunizations and initial TB test, annual physical exam.Attend all meetings and trainings as required.Attend other related meetings and trainings as requested which enhance professional development.Adhere to all plans, policies and procedures of Governors State University, Family Development Center and Early Head Start.Required and Preferred Qualifications

Required

A BA/BS in Child Development, Family Studies, Social Worker or related field with the equivalent of a Home Base CDA credential as a part of degree, or a Level 3 Family Specialist Credential from Illinois Gateway to Opportunity.Must be able to obtain certification in Parents as Teachers Certification, Partners for a Healthy Baby Certification, Teaching Strategies Gold – Interrater Reliability Certification within three (3) months of hire.Must possess excellent oral and written communication skills.Experience in word processing/computers.Must possess a pleasing personality and ability to establish and maintain rapport with families and staff. Must be capable of representing the Family Development Center and University in a positive mannerMust be able to transport enrolled children and families in agency vehicle to and from agency events as needed.Must possess a valid driver’s license, and use reliable transportation on the job.Must possess a clean driving recordMust report all changes in driving record to supervisor and Executive Director of the FDCMust complete and maintain all training, including but not limited to CPR and First Aid certification, Shaken Baby Syndrome, Sudden Infant Death, Mandated Reporter, Trauma Informed Practice
