Job Overview - Care Coordinator:
Looking for a fulfilling role in care coordination? Join our Program in Staten Island, NY, as a Care Coordinator. You will be crucial in managing client care and ensuring they receive the necessary support and services.

Location: Staten Island, NY
Compensation: $25/hour
Schedule: 9 AM to 5 PM, Monday to Friday

Responsibilities as the Care Coordinator:

  • Provide care coordination for clients, ensuring access to necessary services like medical care, behavioral health services, and social support.
  • Advocate for client services via phone and in-person.
  • Conduct home visits and follow-up to monitor client status.
  • Participate in case conferences and training sessions.
  • Maintain relationships with service providers.
  • Keep accurate and timely documentation in client charts and records.
Qualifications for the Care Coordinator:
  • Bachelor's Degree or High School Diploma with 2 years of care management experience.
  • Experience working with homeless and HIV populations.
  • Bilingual skills in Spanish preferred.
  • Exceptional communication skills, both oral and written.
  • Clean valid driver's license required.
