We are searching for a Public Health Worker who understands the needs of the families we serve and is passionate about guiding them to achieve their optimal level of health. G E N E R A L S T A T E M E N T O F D U T I E S Assures the health and social service needs of a specific population are being met primarily through community outreach, visits to other agencies, and direct contact with allied health provider offices. In partnership with a health professional, assists with case management functions. Provides information and education on health-related issues. May act as an interpreter for clients. S U P E R V I S I O N R E C E I V E D Works under the general direction of a health services professional and/or supervisor, who assigns work, establishes goals and reviews the results obtained for overall effectiveness. S U P E R V I S I O N E X E R C I S E D Supervision of other employees is not a responsibility of positions in this class. Typical Duties - Duties include, but are not limited to the following This position provides services to monolingual Spanish-speaking clients; therefore, fluent bilingual Spanish/ English skills are required. Under the direction of a Public Health Nurse ( P H N), provide case management services to Babies First and Ca Coon clients (pregnant and postpartum women, children, and adolescents) on P H N caseload to include: Assist families in accessing needed resources. Provide orientation for new clients regarding benefits available to them. Conduct home visits, provide information, support client access to services Advocate to have client needs met. Collaborate with other community service providers Attend community meetings and report updates to team. Serve as contact person for layettes; track and organize donated items. Place N C O orders Identify potential funding sources for special projects/equipment for families. Provide Oregon Mothers Care ( O M C) services: Maintain certification as Health Care Assister with the Oregon Health Plan ( O H P) and Health Care Marketplace ( Health Care.gov) Assist clients to apply for O H P/ Health Care Insurance and provide follow-up case management of those applications. Provide client support for gaining access to prenatal services. Comply with O M C documentation and Data Reporting requirements. Other duties as assigned by your supervisor. Response Level 2: In the event of an emergency, employee makes a reasonable effort to report to work. Employee may be required to perform duties outside of their regularly assigned duties. Employee may be requested to work an extended or flexible schedule. Staff at this response level must complete Incident Command System 100 and 700 on-line or in-person training. Requirements for the Position E X P E R I E N C E A N D T R A I N I N G Graduation from high school or G E D; A N D Associate degree in social sciences, a health occupation, mental health or related field, with two (2) years experience in community health, medical, mental health, or alcohol/drug services or other related fields; O R Four (4) years in community health, medical, mental health or alcohol/drug services or related advocacy, health education or other related fields with the culture of the population being served; O R Any satisfactory combination of work, education, training, or experience relevant to the position, as determined by Marion County. S P E C I A L R E Q U I R E M E N T S This position provides services to monolingual Spanish-speaking clients; therefore, fluent bilingual Spanish/ English skills are required. An additional 5% will be added to the hourly wage as compensation for this skill. Finalist will be required to pass a verbal and written Standardized Language Fluency Assessment as part of the hiring process. Must possess a current driver's license in the applicant's state of residence and an acceptable driving history. Marion County will obtain a copy of the driving record for all qualified applicants from Driver and Motor Vehicle Services and review the driving record according to the Marion County policy and procedure for Driving on County Business. If personal vehicle is used, must possess and maintain proof of personal automobile liability insurance, at a minimum, in the amount required by Oregon Law [ O R S Chapter 806]. The finalist for this position will be required to pass a criminal history background check; however, conviction of a crime may not necessarily disqualify an individual for this position. P H Y S I C A L R E Q U I R E M E N T S Operates a motor vehicle; drives to homes to visit clients; sees using depth perception; stands; sits; moves about the work area; bends forward; reaches overhead; lifts up, pushes, pulls up to 30 lbs.; moves carts weighing up to 30 lbs.; carries items weighing up to 30 lbs. for up to 20 feet; operates a keyboard; speaks clearly and audibly; reads a 12 pt. font; distinguish colors and shades; uses depth perception; hears a normal level of speech; works in areas that may be exposed to weather, heat, cold, humidity, dust, chemicals, smoke, noise, exposure to cleaning disinfectants, and works in areas that may be uneven. This position may be exposed to screeching or crying infants or children in homes. There is the potential to be exposed to mold, dust, and smoke in client homes.
