Case Manager for the Adult Care Management State Funded program. The Case Manager meets the intensive needs of the population while meeting visit requirements as stipulated by OMH for the Adult Care Management State Funded program. Clients include persons diagnosed with serious mental illnesses and possible additional medical and substance use diagnoses.

The Case Manager will maintain a positive, working rapport with Suffolk County Adult SPOA.
Employ a culturally sensitive and proactive outreach approach to potential members.
Speak with mental health, medical, and substance abuse providers in addition to hospitals, jails, and housing/shelter providers if necessary.
The Case Manager will form and maintain a positive and professional relationship with both internal and external providers in order to coordinate care and obtain needed information.
Report all incidents to the program management staff.
Bachelor’s Degree in a health-related field required.
Proficiency in Microsoft Office required; This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
