Position Summary

Coordinating with the acute care Hematology/Oncology Clinical Specialists covering the hematologic or solid tumor service and the Specialty Pharmacist in the GW MFA Outpatient Pharmacy, this individual will ensure continuity of care as the patient transitions from acute care to the ambulatory care setting.
