Our Industrial Operations program in Fort Wayne, IN is currently seeking a Community Connections Coordinator.

GENERAL RESPONSIBILITIES:

Under the supervision of the Community Connections Manager, the Community Connections Coordinator will provide support to individuals served as they pursue discovery activities out in the community, such as business tours, job shadowing opportunities, volunteerism and job trials.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Support individuals in the community while they are engaging in training and experiences centered around the following activities: Self Advocacy, Career Counseling, Work-Based Learning, Volunteerism and Workplace Readiness.
  • Track data and keep daily reports of client progress.
  • Follow all protocols related to safely taking individuals served into the community.
  • Work with each individual served to identify and explore client vocational interests and communicate this information to the Community Connections Manager
  • Maintains certification in medication administration, CPR, and First Aid.
  • Attend departmental meetings.
  • Stay current on all individuals PCISP, risk plans, and behavior plans.
  • Attendance for work per requirements of program, Benchmark, and client specific needs.
  • Reasonable suspicion of a crime against an ICF/MR resident must be reported to Indiana State Department of Health and Law Enforcement (Elder Justice Act).
  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.
  • Other duties as assigned.
  • Comply with all standards to assure the health and safety of all individuals.

Additional duties of part time Community Connections Coordinator:

  • Assists the assigned Program Coordinator to ensure quality, health and safety and client centered approaches for individuals served in our Industrial Work Centers.
  • Gather Industrial Services information for quarterly and annual ISP/PCISP reviews.
  • Responsible for accuracy and quality of client flies, both electronic files and paper files.
  • Responsible for maintaining contact with Consumers’ interdisciplinary teams, consisting of behavior consultants, families/guardians, and waiver case manager.
  • Responsible for assisting in client specific training of supervisors on client ISP/PCISP.
  • Assist Team Leads to prepare for surveys, state audits, and internal reviews.
  • Attend all scheduled trainings and staff meetings.
  • Meet standards and complete documentation required by state, federal, and accreditation regulations.
  • Be aware and submit reportable incidents within required time frame.
  • Stay current on all individuals ISP/PCISP goals, meds, and behavior plans for the individuals on their caseload.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Must be able to work with minimal supervision (“self starter”).
  • Effective communication skills with staff, management, individuals served, and other internal and external stakeholders
  • Functional knowledge of basic computer skills

QUALIFICATIONS:

  • Experience working with adults with disabilities preferred.
  • High School diploma or equivalent required.
  • Valid driver’s license and proof of auto insurance
