Under the supervision of the Manager and the direction of the Manager, Nursing Support Services, provides direct patient care in the home during crisis or transition period in providing end-of-life care/support for patients and families alike. Provides intermittent visits as needed to support patients in their home or LTC setting.
All employees assigned this job title must be willing to travel anywhere within our organization’s service area and to flex their work location within the service area and between any Community Hospice and Palliative Care location, General Inpatient Unit, or team at any time if there are no Crisis Intervention Care cases on assigned days of work.

PRIMARY DUTIES AND RESPONSIBILITIES:

Completes assessment of patients regularly and on-going as patients’ conditions warrants.Provides patient care in accordance with physician orders and patient/family goals, directed by the Care Plan; provides post-mortem care per policy.Demonstrates awareness and maintenance of Universal Precautions.Provides emotional support to patient/caregiver.Teaches patient/caregiver as appropriate to situation, plan of care.Demonstrates knowledge and appropriate use of medications, including medication administration.Demonstrates competency in skills identified, maintaining completion of annual competency of these skills per agency standard.If Crisis Intervention care case ends, notify supervisor immediately.Give Shift Report to Team and Manager of CIC.Call time in at the end of every scheduled shift.Follows company policies and guidelines related to specific job functions.Other duties as assigned.

Coordinates implementation of Plan of Care so that:

Communicates timely, thoroughly, professionally with Medical Director, Home/LTC Team Manager/designee, Inpatient unit team members (as appropriate), Home and LTC teams, patient/family, other Community Hospice departments.Evaluates services provided to patients, apprising Inpatient Unit Manager /designee if compliments/concerns or opportunities for improvement are expressed.Facilitates transfers into the Inpatient Facility (if appropriate) further ensuring continuity of care of Hospice services.

Completes documentation so that:

Charts patient condition/changes every 1 hour during shift. (For CIC cases only).Documentation completed is reflective of patient care given, services provided, relevant to Continuous Care criteria and death/discharge process. Any concerns noted, patient status; is legible, neat; is accurate, thorough, completed in a timely manner.Documents relevant to the Medication Administration, Narcotic Administration, and accountability and destruction of medication following patient death.Care Plans are kept current, applicable to patient and patient needs.Incident Reports are completed accurately and submitted immediately to the Nursing Support Service Manager/designee.

Participates in total agency program so that:

Demonstrates professional relationship with internal and external customers.Promotes agency image within Community Hospice and the community.Uses chain of command for problem solving.Demonstrates appropriate dress and grooming.Displays courteous, tact, patience, respect.Maintains a good attendance record; notifies agency of absences according to policy.Participates in agency Quality Assurance as appropriate.Follows and supports the Clinical Care standards as established.

Actively seeks ways to develop professionally by:

Developing basic knowledge of Hospice Medicare/Hospice Medicaid and other reimbursement mechanisms.Developing areas of expertise in hospice nursing skills.Pursuing professional growth through academic and continuing education programs.Assisting in the orientation and mentorship of new employees.

QUALIFICATIONS:

REQUIRED LICENSES/CERTIFICATIONS:

Current State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier.Valid LPN LicenseProof of IV CertificationCurrent BLS

EDUCATION/EXPERIENCE:

Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable.

Typical qualifications would be equivalent to:

Graduate from an accredited Practical Nursing ProgramMinimum of one (1) year experience in acute care setting, long term care setting or home health setting as an LPN. Reliable transportation that allows employees to shift work locations when needed.Furnish one or more active telephone numbers that enables management to contact employee should an unexpected change of work location be required.

KNOWLEDGE/SKILLS/ABILITIES:

Appropriate educational principles and methods as applied to hospice nursing.Effective communication and listeningDemonstration of confidence and ability to manage inpatient facility functions and patient care needs on his/her shiftWorking long and occasionally irregular hoursWorking with the framework of a multidisciplinary team

MEDICAL CLEARANCE:

Must have medical clearance for communicable diseases before direct patient contact.Current immunizations including but not limited to TB, Hepatitis, and Flu.
