Job Title: LPN Crisis Intervention Care Specialist

Community Hospice of Northeast Florida is seeking a skilled LPN Crisis Intervention Care Specialist to join our team. As a Crisis Intervention Care Specialist, you will provide direct patient care in the home during crisis or transition periods, providing end-of-life care and support to patients and their families.

Responsibilities:
  • Complete assessments of patients regularly and as needed, providing patient care in accordance with physician orders and patient/family goals.
  • Provide emotional support to patients and their caregivers, teaching them as needed to ensure a smooth transition.
  • Coordinate implementation of the Plan of Care, communicating effectively with medical directors, home/LTC team managers, and other healthcare professionals.
  • Document patient care and services provided, ensuring accurate and timely completion of charts and reports.
Requirements:
  • Current State of Florida driver's license with a driving record acceptable to Community Hospice insurance carrier.
  • Valid LPN license and proof of IV certification.
  • Current BLS certification.
  • Minimum of one year experience in acute care, long-term care, or home health settings as an LPN.
