Long term care skilled nursing facility located in Jersey City, NJ is looking for an experienced and knowledgeable MDS Assessor

MDS Assessor-Nursing Home

*Salary: $85K per year (depending on experience)*

Requirements of the MDS Assessor-Nursing Home:

  •          Must have experience as an MDS Assessor

    ·         Must have at least 2 years’ experience in a skilled nursing facility working in MDS

    ·         Must have NJ RN license

    ·         Must have long term care experience

    ·         Must know MDS 3.0

Responsibilities of the MDS Assessor-Nursing Home:

  • Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
  • Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
  • Provide clinical support and direct to appropriate resource materials.
