Key Responsibilities:
- Monitor and audit clinical records to ensure accuracy and timeliness.
- Review documentation and make recommendations to improve patient care.
- Assist with chart audits to ensure compliance with regulatory requirements.
- Prepare, file, and track electronic paperwork submissions.
- Ensure the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process.
- Prepare assessment schedules and attend care plan assessment meetings.
- Complete all required documentation for medical records.
- Collaborate with residents, families, and the interdisciplinary team to ensure comprehensive patient care.
- Review clinical records for accuracy and compliance with MDS coding.
Requirements:
- Active RN license from the state of New Jersey.
- Experience in long-term care and at least one year of job-related experience in the MDS process.
- Expertise in working with MDS 3.0 and PDPM.
- Strong experience with Medicaid and Medicare - CMI scoring.
- Familiarity with various computer software.
- Efficient, organized, detail-oriented, and flexible.
- Self-directed but team-oriented.
