Key Responsibilities:

  • Monitor and audit clinical records to ensure accuracy and timeliness.
  • Review documentation and make recommendations to improve patient care.
  • Assist with chart audits to ensure compliance with regulatory requirements.
  • Prepare, file, and track electronic paperwork submissions.
  • Ensure the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process.
  • Prepare assessment schedules and attend care plan assessment meetings.
  • Complete all required documentation for medical records.
  • Collaborate with residents, families, and the interdisciplinary team to ensure comprehensive patient care.
  • Review clinical records for accuracy and compliance with MDS coding.

Requirements:

  • Active RN license from the state of New Jersey.
  • Experience in long-term care and at least one year of job-related experience in the MDS process.
  • Expertise in working with MDS 3.0 and PDPM.
  • Strong experience with Medicaid and Medicare - CMI scoring.
  • Familiarity with various computer software.
  • Efficient, organized, detail-oriented, and flexible.
  • Self-directed but team-oriented.
