Job Summary:

We are seeking an experienced and knowledgeable MDS Assessor to join our team at our skilled nursing facility in Jersey City, NJ. As an MDS Assessor, you will play a critical role in ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments, which are used to determine the level of care required by our residents.

Responsibilities:

  • Conduct thorough and accurate MDS assessments to ensure the highest quality of care for our residents.
  • Collaborate with the interdisciplinary rehab team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Review and analyze resident data to identify trends and areas for improvement, and make recommendations to optimize care delivery.
  • Provide clinical support and education to nursing staff and other healthcare professionals to ensure they have the knowledge and skills needed to provide high-quality care.

Requirements:

  • Minimum of 2 years of experience as an MDS Assessor in a skilled nursing facility.
  • Current RN license in the state of New Jersey.
  • Experience working in long-term care and knowledge of MDS 3.0.
