The Healthcare Coordinator (HCC) links adults, children and their families to comprehensive medical, behavioral health and psychosocial services to promote and improve the overall health of the individual as well as the family. This may include but is not limited to psychoeducation, care coordination, care navigation and advocacy for patients in need of or receiving medical care; supporting integrated behavioral and medical collaborative care plans; monitoring the patients’ and families’ receipt of services and identifying and reducing barriers to services. This position works closely with collocated Integrated Care Clinicians and the medical care teams, Jefferson Center prescriber and care team, and staff of other organizations involved with and in the patient’s and family’s care. This position will be office/community clinic based, hybrid options are available and will include clinical work and administrative duties; 10-hour workdays are an option with this role. This role will serve the residents of Marisol Homes in Lakewood.

Essential Duties:

Assesses barriers to health, determines appropriate service referrals, determines appropriate ways to address those barriers, ensures services are received and monitors results Researches, prepares materials, and provides education and support as needed to clients. Coordinates the delivery of services to patients, including collaborating with clinical and other staff to address patients’ needs, as appropriate Facilitates information exchange and processes continuity of care documents for clients Creates Avatar charts, coordinates care with clinician, insurance and integrated practice for clients with commercial insurance Connects patients to primary and specialty care resources within the community to address identified health concerns Participates in case review sessions. Provides timely documentation and required reports for all care coordination activities. Service delivery systems include coordination of referral systems, and interfacing with internal clinical and non-clinical services and programs Provides input into care coordination protocols and procedures based on evidence-based models Research and provide guidance on outreach programs for a diverse population of at-risk clients Attends all staff meetings and all other meetings as assigned Reports high risk/problem cases, and utilizes a problem-solving approach as well as feedback Attends supervision at times and intervals agreed upon with supervisor

Education, Knowledge, Skills & Experience Required:

Completed Bachelor's Degree in Social Work, Psychology, Public Health or related field required.

2+ years’ experience in case management or health care navigation in a medical care setting required.

Proficient skills in care coordination, health care navigation, charting, documentation, collaboration, and advocacy.

Bilingual desired (Spanish/English)

Salary Grade 11- $44,900 to $57,100.*

Additional Salary Information*:

All salaries at Jefferson Center are calculated based on years of total relevant experience.* The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*

*This position qualifies for a limited time grant funded bonus of $1000 paid on the one month anniversary *

Additional Bonus Information*:

Based on full time (40 hours/week). Prorate if less than 40 hours/week* Must work a minimum of 20 hours/week to be eligible for bonus* Current Jefferson Center employees not eligible for sign on bonus*
